Does Kindle for PC have a built-in TTS engine? How do you make it read the text of an ebook I downloaded? I bought it from Amazon so it's not like it's some downloadable file I obtained from an alternate website.
Here is a link on Using the PC version with TTS. You need to have additional software installed.
Ok. I installed it. How do I actually use it once it's installed?
Follow the instructions here http://www.amazon.com/gp/help/customer/display.html/?nodeId=200596280&pop-up=1. Start Microsoft Narrator before using the keyboard shortcut.
I installed the additional software, and I opened an ebook and I hit CTRL + R. It doesn't work for some reason since I don't hear anything being read.
It appears based on my research that you are referring to the Send to Kindle Google Chrome extension as seen at http://www.amazon.com/gp/sendtokindle/chrome/ Is that correct? Are you wanting your Kindle Fire or older device to read what you are sending to your device once it arrives? If so then the following articles will help you solve the issue; I have no idea which Kindle device you are using. Good luck.

I'm not using the actual Kindle device. I have a chrome extension called Kindle Cloud Reader and I am running into some setbacks while trying to make this app read the book to me aloud.
Take a look at http://www.addictivetips.com/windows-tips/chrome-speak-select-text-from-any-webpage-to-read-out-loud-chrome/ and see if it might hold an answer for you. I had forgotten about Kindle Cloud Reader which will work in any desktop browser. Hopefully this is more along the lines of what you need. Good luck.
I've tried it with webpages such as Wikipedia articles, but on the Kindle cloud reader it doesn't work, for some odd reason.
If you can’t read a book because you’re driving or for some other reason, both the second-generation Kindle and the Kindle DX have an “experimental” feature that converts any text to speech and reads it to you. Calling a feature experimental means that it’s a peripheral Kindle feature that Amazon is working on; they’re available for Kindle owners to use but they might not be fully featured. These are also features that Amazon could choose to discontinue.

Despite the “experimental” label, these are some of the most useful and intriguing features of the Kindle. One of these is the Text-to-Speech feature. It enables your Kindle to read books, newspapers, blogs, or other text to you.

The voice begins to talk. It’s remarkably human sounding for a computer-generated voice. You might find that the monotone expression and speed make the voice difficult to understand, however.

You can adjust the volume by pressing the Volume Up or Volume Down button on the side of your Kindle.
You can also adjust the speed at which the text is read, and choose either a male or female voice. These options (shown in the figure that follows) are available when you press the Aa key on the keyboard.
You can listen to speech either with headphones or through the Kindle’s speaker. It’s the perfect option for long car rides. If your car’s audio system has an input jack for audio, you can connect the Kindle to it with a standard audio plug. Just plug one end of the cable into the Kindle’s headphone jack and the other into your car stereo’s input jack, and you can hear the content read to you over your car’s speakers.
